Schedule: Full Time| 80 Hours/Pay Period | Monday to Friday - 8 a.m. to 4:30 p.m.
Pay range: $45.00/hour to $56.50/hour (hourly non-exempt) based on qualifications, experience, and location.
As a Physical Therapist, you'll be at the heart of our community, providing hands-on therapeutic support to clients while fostering meaningful connections. Whether creating personalized treatment plans, guiding clients through rehabilitation exercises, or collaborating with an interdisciplinary team, you'll play a key role in helping clients achieve their highest level of independence and well-being.
As a Physical Therapist, you will:
Deliver Direct Client Care - Conduct comprehensive evaluations and develop personalized treatment plans with short-term and long-term goals.
Maintain Accurate Client Records - Document services accurately and timely using Vivie's EMR system, ensuring compliance with all payer sources and authorization requirements.
Facilitate Follow-Up Programs - Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes.
Collaborate and Communicate Effectively - Work closely with clients, families, physicians, and the interdisciplinary team to provide care.
Ensure Regulatory Compliance - Complete and document supervisory visits with PTAs and RA/CNAs in accordance with state and regulatory guidelines.

This job also requires:
Current Physical Therapist license in the state of Minnesota.
Strong collaboration and teamwork skills.
Clear and compassionate communication abilities.
Proficient in English for effective communication with staff, clients, and visitors.
