Full Job Description
Physical Therapist;

Range: $43.00 - $47.00

Position Summary: The Physical Therapist assesses, plans, organizes, and administers rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.

Job Qualifications:
  • Education: Graduate of an accredited therapy program
  • Experience: 2-3 years of related experience; or equivalent combination of education and experience (preferred).

Requirements

Core Purpose and Assessment: Restores patient's function, alleviates pain, and prevents disabilities through planning and administering medically prescribed physical therapy; provides quality care by assessing and interpreting evaluations, test results, mobility, strength, and current function levels to set realistic, attainable goals scaled to patient abilities.

Treatment Planning: Develops individualized Plans of Treatment based on initial and ongoing evaluations, including short- and long-term goals, collaboration with staff, and patient/caregiver involvement; determines plans in consultation with physicians or by prescription.

Implementation: Administers treatment plans on a consistent schedule, adjusting based on patient progress; helps patients achieve goals via manual exercises, massages, and necessary equipment.

Ongoing Evaluation: Evaluates and records progress, modifies treatment plans, and incorporates new treatments as needed; assesses prosthetic/orthotic devices for use, fit, progress, and recommends adjustments/modifications.

Documentation and Reporting: Documents patient care services per approved medical and facility procedures; reports progress to patients, caregivers, loved ones, facility staff, and involved healthcare professionals.

Discharge Planning: Develops post-treatment plans with appropriate in-home care services upon completion of the Plan of Treatment.
