Requirements
Current NYS Physical Therapy license
New York State Education Department license
Age-appropriate experience
Ability to provide English services
Mandatory Clearance Requirements
All providers must be cleared through NYCPS Office of Related Services (ORS) prior to starting
Providers must submit resume, licenses, certifications, availability, and provider SSN via secure Stafferlink collection process
Individual provider applications will be coordinated directly with ORS
No start date permitted before ORS clearance and RSA authorization
