School-Based Physical Therapist - Brooklyn, NYShift & Schedule Details
- Shift hours: 7:55 AM - 2:35 PM
- Location: Brooklyn, NY 11237
- Approximate weekly onsite hours: 5.5 hours
- Sessions are 30 minutes each
- Exact hours and session duration to be confirmed in interview
Caseload Information
- 5 individual students / 9 individual sessions per week
- 1 group student / 1 group session per week
- Population: Middle School
Position Overview
- On-site, school-based Physical Therapist role
- Deliver individual and group IEP services
- Document student progress
- Collaborate with school personnel
- Open school-based PT caseload
Requirements
- Current NYS Physical Therapy license
- New York State Education Department license
- Age-appropriate experience
- Ability to provide English services
Mandatory Clearance Requirements
- All providers must be cleared through NYCPS Office of Related Services (ORS) prior to starting
- Providers must submit resume, licenses, certifications, availability, and provider SSN via the secure Stafferlink collection process
- Provider applications will be coordinated directly with ORS
- No start date permitted before ORS clearance and RSA authorization are obtained
