ResponsibilitiesJob SummaryThe Physical Therapist evaluates and treats patients with musculoskeletal injuries, conditions and disabilities through the use of various physical modalities, therapeutic exercises and therapeutic procedures in accordance with the physician's orders and department policies.
Essential Functions- Assesses, consults with physician, develops plan of care and goals with Provides treatment based on physician's orders.
- Utilizes electronic health records efficiently and effectively to complete evaluations, daily notes and physician progress reports in a timely manner.
- Observes and reassesses the patient's condition and recommends appropriate changes to the plan of care.
- Instructs patients in home exercise
- Operates within safety, infection and environmental control policies and procedures for physical therapy.
- Fits, instructs in use and dispenses assistive devices appropriately based on patient's needs to ensure safe ambulation and transfers.
- Fits and instructs patients in the use and care of durable medical equipment as prescribed by the physician, and accurately completes paperwork for billing.

- Stays current in their respective field through reading professional literature and attending continuing education offerings, etc.

- Supervises and reviews physical therapy assistant treatments and Co-signs and submits fee slips accurately and in a timely manner.
QualificationsStandard Qualifications:Education: B.S., M.S., or Ph.D in an accredited physical therapy program. Registered as a physical therapist and licensed in the state of Indiana.
Experience: Minimum of two years physical therapy experience preferred. New graduates are encouraged to apply.
