We're looking for a full-time Physical Therapist to serve patients in Sonoma County. This individual would be based out of their home while traveling to and from patient homes for clinical care.

Key Responsibilities

  • Evaluate patients' functional status, muscle strength, mobility, gait, and ROM to develop and direct individualized treatment plans
  • Instruct patients, families, and caregivers on therapeutic appliances, PT goals, and relevant care phases
  • Monitor and report patient progress and condition changes to the physician
  • Prepare and submit clinical progress summaries and participate in discharge planning
  • Identify needs for additional home health services and refer accordingly
  • Supervise Physical Therapy Assistants and, when PT is the sole skilled service, evaluate home health aide care every two weeks
  • Provide consultation, staff in-services, and participate in peer review as needed

  • Strong verbal and written communication skills
  • Current CPR certification
