Position OverviewAs an In-Home Outpatient Physical Therapist Assistant, you'll work under the direction of a licensed Physical Therapist to provide skilled outpatient rehabilitation services under Medicare Part B. You'll help patients recovering from orthopedic procedures, neurological conditions, chronic illnesses, and age-related mobility challenges improve their function and safely remain active in their homes and communities.
What You'll Do- Provide skilled physical therapy treatments under the supervision of a licensed Physical Therapist
- Implement individualized treatment plans designed to improve strength, balance, mobility, endurance, and functional performance
- Assist patients with gait training, transfers, therapeutic exercise, and functional mobility
- Educate patients and caregivers on home exercise programs, safety strategies, and adaptive techniques
- Monitor patient progress and communicate clinical updates to the supervising Physical Therapist
- Collaborate with physicians, therapists, and other healthcare professionals to support optimal patient outcomes
- Complete timely, accurate electronic documentation in accordance with Medicare Part B and agency standards
- Deliver compassionate, patient-centered care that promotes long-term wellness and independence
What We're Looking For- Associate degree from an accredited Physical Therapist Assistant program
- Current Connecticut Physical Therapist Assistant license
- Outpatient, home health, or rehabilitation experience preferred, but not required
- Strong communication, organization, and interpersonal skills
- Ability to work independently while collaborating with a multidisciplinary rehabilitation team
- Reliable transportation and a valid driver's license
- Passion for helping patients achieve their highest level of function
