Physical Therapist AssistantMultiple Full Time Positions AvailableTerritory:Targeted and manageable area of San Francisco. We also have ongoing needs in San Mateo, Santa Clara and Alameda Counties.
What you'll do:Under the supervision of a Physical Therapist, you will assist in providing appropriate aspects of quality patient care and address the needs of individuals in their homes, so they can live safely and productively in their home environment. You will:
- Examine each individual and develop a plan using treatment techniques to promote the ability to move, reduce pain, restore function, and prevent disability.
- Work with patients to prevent the loss of mobility before it occurs by developing fitness- and wellness-oriented programs for healthier and more active lifestyles.

To be a success:- License PTA in California
- Home Health experience is highly preferred
- Current CPR License
- Current CA Driver's License
- Computer Proficiency
- Must be willing to travel within a 10-20 miles radius to see patients.
- Strong organization and communication skills
- Work well independently and in a team
- Excel at building rapport with patients and providing the highest quality of care
- Experience with high acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
- Case Management fundamentals is highly desired

Wage Range $70 - $90 Pay Per Visit Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers comprehensive health benefits and 401k (Up to 4% match)
