Compensation Details:

• $53 per point therapy services. Mileage is paid at $0.43 per mile. Milage is not paid on travel for the first or last visit of your workday. OASIS assessments pay $45 per point.

• OASIS Discharge is 1.56 points per visit

• OASIS Non-Billable Discharge is 0.45 points per assessment

• OASIS Recertification 1.56 points per visit

• PT Discharge 1.5 points per visit

• PT Resumption 1.5 points per visit

• PT SOC 1.5 points per visit

What You'll Be Doing

As a Physical Therapist, you'll provide individualized therapy in patients' homes and help them work toward safer, stronger, and more independent movement.

You'll:

• Evaluate patients and identify their physical therapy needs
• Develop treatment plans based on each patient's condition and goals
• Work on strength, mobility, balance, coordination, and functional movement
• Provide hands-on therapy and therapeutic exercises
• Monitor patient progress and adjust treatment as needed
• Teach patients and families exercises and techniques they can continue at home
• Help patients improve their ability to safely move around their homes and communities
• Recommend appropriate equipment and mobility aids when needed
• Communicate with physicians, families, caregivers, PTAs, and other members of the care team
• Complete accurate documentation of evaluations, treatments, progress, and follow-up care
• Participate in discharge planning and quality assurance activities
• Supervise and provide direction to Physical Therapy Assistants (PTAs) when required
• Provide education to patients, families, caregivers, staff, and the community

What We're Looking For

We're looking for a Physical Therapist who is:

• Licensed as a Physical Therapist in Ohio
• A graduate of an approved Physical Therapy program
• MS/DPT in Physical Therapy or BS with equivalent qualifying experience
• Successfully completed the required state certification examination
• Current with CPR certification
• Committed to continuing education and professional development
• A confident communicator with good interpersonal skills
• Comfortable working independently with minimal supervision
• Dependable, organized, adaptable, and patient-focused
• Comfortable working with patients with physical, emotional, or cognitive challenges
• Comfortable using computers for documentation and communication

The Details

This position involves frequent travel for home visits. Candidates must have:

• A valid Ohio Physical Therapist license
• Current CPR certification
• A valid driver's license
• A safe and reliable vehicle
• Current automobile insurance meeting Ohio requirements
• A working phone and valid email address
• Ability to safely perform patient transfers and other physical aspects of the position, with reasonable accommodation when applicable
