Responsibilities

  • Develop and maintain antimicrobial stewardship programs for market facilities
  • Manage market level Pharmacy and Therapeutics (P&T) meetings
  • Educate staff regarding intervention documentation
  • Analyze intervention documentation for accuracy and appropriateness
  • Lead quality improvement, risk management, and patient safety initiatives
  • Research and report on adverse drug reactions and medication errors
  • Oversee clinical pharmacy training programs and support residency programs if applicable

Full Job Description
POSITION SUMMARY

The Market Pharmacy Manager is responsible for designing, implementing and managing the pharmacy clinical programs across all acute care settings within the designated market. This includes clinical program design and implementation, formulary management, policy and procedure development, key market clinical meetings and education programs across the market. Additionally, they will indirectly support, mentor and oversee the clinical activities of the pharmacists within the facilities.

Responsibilities

  • Develops, manages and maintains primary responsibility for pharmacy leadership of antimicrobial stewardship programs for the market, highlighting needs and opportunities for each facility within the market.
  • Develops, manages and maintains primary responsibility for market level P&T meetings.
  • Responsible for oversight and education of staff regarding intervention documentation.
  • Responsible for ongoing analysis of intervention documentation to ensure accurate and appropriate documentation of clinical activities.
  • Leads initiatives aimed at achieving the goals of the organization's quality improvement, risk management and patient safety programs to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related
  • Participates in the research, analysis and reporting of adverse drug reactions, medication errors and pharmacy quality initiatives; identifies areas of improvement and actively leads improvement projects based on findings.
  • Oversees clinical pharmacy training programs when present.
  • If a residency program is present, must support the program through responsibility for or assistance with designing and managing residency program and mentoring student/resident as well as mentoring preceptors and providing scheduling support for preceptors and residents to achieve the program requirements.
  • Identifies, designs, collects data and analyzes drug utilization reviews to promote rational drug therapy and determines action plan based on
  • Effectively and proactively interacts with medical staff regarding drug therapy issues, initiatives, challenges and works to identify optimal resolutions ensuring continuity of quality patient care.
  • Works with the Pharmacy leadership team to identify and develop processes to meet or exceed all regulatory standards and participates in quality and regulatory reviews and audits; leads and assists with the execution of these processes.
  • Sustains the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promotes rational drug therapy selection.

Qualifications

Job Requirements:
  • Bachelor of Science or Doctor of Pharmacy degree from an ACPE-accredited School of Pharmacy.
  • 1+ years as a Pharmacy Clinical Specialist (or equivalent experience)
  • Clinical pharmacy experience in a hospital setting
    • Completion of a PGY-1 Pharmacy Residency or fellowship AND
    • Completion of a PGY-2 Specialty Pharmacy Residency or fellowship (in an appropriate specialty field)
    • OR Completion of a PGY-1 Pharmacy residency or fellowship AND
    • 3 years clinical pharmacy experience (if no PGY-2 is completed) OR
    • 5 years clinical pharmacy experience with no history of residency or fellowship
  • Experience working in Microsoft Office applications, Electronic Medical Record Systems.
  • Active Pharmacist license in good standing with the appropriate State Board of Pharmacy.
  • Other applicable state requirements- i.e. sterile compounding, etc.
  • BLS certification.


Preferred Job Requirements:
  • Hospital Pharmacy Management experience.
  • ACLS certification.
