Pharmacy Clinical Coordinator & Pharmacist in Charge (PIC) - Full Time

Shift: Days, 8:00am - 4:30pm

Job Description:

PATIENT POPULATION:
The patient population served is all patients, including geriatric, adult, adolescent, pediatric, and newborn. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.

POSITION SUMMARY:
Under the direction of the Department Leadership, provides clinical pharmaceutical care to patients and serves as expert, professional resource to service areas and the Department of Pharmacy Services. Serves as the Pharmacist in Charge and provides direct supervision and oversight of inventory and purchasing. Incumbent pharmacist-in-charge shall be responsible for a pharmacys compliance with all state and federal laws and regulations pertaining to the practice of pharmacy.

Must be able to work normal/scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act.

EDUCATION/TRAINING/EXPERIENCE:

One (1) year of experience with information technology related to medication management safety is strongly preferred. To perform this job successfully, an individual should have advanced clinical skills, successful interpersonal relations, and increased responsibility. Must have and maintain current licensure as a Registered Pharmacist in the state of California. Doctorate preferred. Residency preferred.

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence if required. Ability to read, write, speak and understand English, and communicate effectively verbally and in writing.

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, and percentages if required.

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

To perform this job successfully, an individual should have knowledge of basic computer skills.

LICENSURE/CERTIFICATIONS:
To perform this job successfully, an individual must have and maintain current licensure as a Registered Pharmacist in the state of California. Must have and maintain current American Heart Association BLS certification or obtain within orientation process, prior to assignment.

Responsibilities and Essential Functions:
*Indicates Essential Function

1 * Assists the department in the creation and maintenance of clinical programs and services.
2 Participates in all committee functions that are pertinent to patient care and medication issues.
3 * Serves to guide the creation and maintenance of pharmacy-related content within the electronic health record to include order set creation and maintenance.
4 * Serves as a subject matter expert for the ACS department in IT Pharmacy applications and projects.
5 * Develops and/or facilitates the development of action plans related to Clinical Pharmacy activities and IT Pharmacy projects.
6 * Demonstrates ability to foster good relations with the Interdisciplinary Team while still performing necessary duties as a Registered Pharmacist.
7 * Manages IT Pharmacy oriented applications such as the Carefusion Knowledge Portal, Alaris CQI, Alaris Drug Library and Sentry 340B Software.
8 * Assists the department in the creation and maintenance of policies and procedures that affect Pharmacy operations and patient care.
9 Models appropriate communication and assists other staff in the identification of appropriate communication channels and methods.
10 * Equips the department of pharmacy with reporting and data collection tools within the Meditech environment.
11 * Rresponsible for a pharmacys compliance with all state and federal laws and regulations pertaining to the practice of pharmacy.
