Overview

Seeking a Long-Term Care Pharmacist to manage medication therapy for patients in nursing homes, assisted living, and hospice facilities. This teammate will balance both dispensing duties and clinical consulting, focusing on geriatric pharmacotherapy to optimize patient safety, minimize adverse events, and ensure regulatory compliance.

Key Responsibilities
  • Support and manage pharmacy technicians as required and as necessary
  • Assist with patient phone calls as necessary
  • Systematically review patient charts to identify potential drug interactions, duplicate therapies, or inappropriate dosages.
  • Oversee the filling, dispensing, and delivery of medications, often using specialized compliance packaging (like multi-dose cards or strip packaging) tailored to facility routines.
  • Ensure all medication administration, storage, and disposal processes comply with state and federal regulations (e.g., Medicare Part D, CMS guidelines).
  • Work closely with facility physicians, nurses, and dietitians to manage chronic conditions, de-prescribe unnecessary drugs, and support antibiotic/opioid stewardship programs.
  • Provide clinical guidance to nursing staff on medication administration and educate residents and their families on treatment outcomes.

Qualifications
  • Comply with State Board of Pharmacy rules and regulations at all times
  • Maintain required Continuing Education credits
  • Maintain current licensing by the state(s) in which he/she practices

Experience
  • 1 year of experience as a Pharmacist minimum
  • IV experience preferred
  • Long term care pharmacy experience is a plus
