Qualifications

• Doctor of Pharmacy (PharmD) degree required.
• Valid Pennsylvania Pharmacist License, including Injectable License for vaccine administration.
• Familiarity with Outcome Platforms essential.
• Medication Therapy Management (MTM) certification preferred.
• Board Certified Ambulatory Care Pharmacist (BCACP) preferred.
• Current CPR certification mandatory.
• Valid PA Driver's License with clean driving history required.
• Spanish language skills beneficial.

Responsibilities

• Provide pharmaceutical care and medication therapy management.
• Administer vaccinations and maintain proper documentation.
• Collaborate with healthcare teams to optimize patient outcomes.
• Monitor and evaluate patient response to medication therapy.
• Educate patients on medication use and potential side effects.
