OverviewPharmacist- InpatientAllen HospitalFull-time & Part-time options available Hours: Primarily 1st shift, rotating evening and weekends, occasional 3rd shift coverage Waterloo, IowaThe Pharmacist shall exhibit professional and technical skills to provide clinical support and drug distribution services in coordination with physician and nursing services for delivery of quality and cost-effective drug therapy for patients. Responsible, professional, and courteous communications (verbal and written) are constant to perform duties and to promote departmental and hospital teamwork for quality patient care. Provides services including but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Works directly with physicians, other health professionals and patients to provide all aspects of patient care, including clinical and distributive tasks, for patients served within our health system. Helps patients and healthcare professionals achieve clinical outcomes and monitor progress toward common goals. Supervises other pharmacy staff in the absence of formal departmental leadership. Performs assignments and projects as directed by the Pharmacy Leadership Team.
Responsibilities- Practices pharmaceutical care.
- Conducts prospective drug use review or evaluates a patient's medication record for purposes of promoting therapeutic appropriateness and ensuring rational drug therapy.
- Provides, documents, and retains a record of the final verification for the accuracy, validity, completeness, or appropriateness of a medication order prior to delivery of the medication.
- Provides patient counseling, dosing consultation, or patient-specific drug information.
- Responsible for all functions performed in the practice of pharmacy. Maintains responsibility for any and all delegated functions including functions delegated to pharmacist-students, pharmacy technicians, and pharmacy support persons.
- Ensures the legal operation of the pharmacy, including meeting all inspection and other requirements of state and federal laws, rules, and regulations governing the practice of pharmacy.
- Maintains records of all transactions of the pharmacy necessary to maintain accurate control over and accountability for all drugs as required by applicable state and federal laws, rules, and regulations.
- Complies with policies and procedures for all operations of the pharmacy.
- Establishes and maintains effective controls against the theft or diversion of prescription drugs, including controlled substances, and records for such drugs.
- Utilizes an ongoing systematic program of continuous quality improvement for achieving performance enhancement and ensuring the quality of pharmaceutical services.
QualificationsEducation:- Graduate of accredited College of Pharmacy required.
- Pharm.D. Residency preferred.
Experience: - One year of hospital experience or Residency preferred.
License(s)/Certification(s): - Current licensure in good standing to practice as a Registered Pharmacist in Iowa or licensure eligible in Iowa required.
- Valid driver's license when driving any vehicle for work-related reasons required.
