Job Specifics

Location: 744 S Webster Ave, Green Bay, WI 54301
FTE Status: Full-Time 1.00 FTE
Work Schedule: Days and PMs rotations (starting at 6:00 am, 7:00 am, or 12:00 pm) with every 3rd weekend and holidays


Job Description:

Responsible for reviewing patient medications during transitions in care, medication therapy management, application of therapeutic interchanges, drug utilization review, efficient drug distribution processes, computerized provider order entry (CPOE) order review and verification, medication checking prior to dispensing, medication drug error prevention, staff development and training, and maintaining a safe work environment. Management of pharmacy quality metrics for at risk payer agreements.

Qualifications:

Wisconsin Pharmacist license required.

Acute care/hospital experience preferred.
