As a member of our pharmacy team, you will play a crucial role in providing exceptional, patient-centered care and exceeding patients' expectations. We are looking for friendly, knowledgeable, and professional individuals who will make meeting the health needs of our patients their top priority.

Here's where you'll have an impact:
    • Demonstrate warmth, empathy, and compassion while playing a key role in achieving excellence in the areas of patient centered care, customer service, workflow efficiencies, corporate pharmacy programs and contractual requirements of continuing care facilities based on corporate agreements
    • Responsible for the provision of clinical services focused on continuing care contractual obligations (i.e., disease state management, medication reviews, full scope of practice, after-hours emergency service
    • Initiate care plans, follow up, and complete proper documentation
    • Identify actual and potential drug related problems through completion of medication reviews
    • Communicate regularly with other members of patient care team including family, nurses, physicians and nurse practitioners
    • Provide education to nursing staff
    • Participate on facility committees and complete biannual medication reviews as part of the care team with Nurses, Physicians, and Nurse Practitioners
    • Ensure prescription safety prioritizing delegating technical functions to Pharmacy Registered Technicians and assistants as required
    • Learning and being familiar with the necessary process needed for accurate automation
    • Learning and being familiar with additional pharmacy software (Custom Health, Point Click Care, Pacmed)
    • Support onboarding new nursing home facilities to our pharmacy and executing eMAR go lives with the nursing team at partner nursing home facilities
    • Support and manage daily workflow in the dispensary (utilizing Kroll and Sharepoint)
    • Complete medication reconciliations for all new admissions/readmissions to partner facilities
    • Support off site vaccination clinics and perform immunizations for LTC residents and nursing home staff
    • Provide quarterly reports to facility administration, review all medication errors and complete audits (stat box supplies, antibiotic use, antipsychotic use, and wound care)
    • Support nursing staff with all pharmacy process and medication related questions
    • Maintain current knowledge of pharmacy practice, regulatory requirements, and industry trends/developments; attend training as required
    • Adhere to all regulatory requirements, company policies, processes, and procedures (e.g., OHS, Loss Prevention, POA)

What you bring to the table:
