Title: Pharmacist

Reporting Relationship: Pharmacy Manager

General Summary of Responsibilities: The pharmacist acts as a drug information resource to patients, medical staff, nursing staff, and ancillary department staff. The pharmacist compounds and dispenses prescribed medications and other pharmaceuticals for patient care by performing the related duties.

Essential Job Functions:

1. Provides pharmaceutical care by collaborating with physicians, nurses, and other members of the healthcare team.

2. Reviews and monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.

3. Discusses medication order clarifications with the prescriber, documents any pharmacy records, and informs others of medication order changes. Provides pharmacokinetic consultations and other dosing as required.

4. Participates in care conferences and swingbed rounds.

5. Assists with medication reconciliations.

6. Promotes the use of the formulary by converting non-formulary orders to formulary when possible, but coordinating procurement of non-formulary drugs when necessary.

7. Provides patient education regarding drug therapy.

8. Participates in the drug therapy management of medical emergencies. Responds to Code Blue emergencies and prepares medications as needed.

9. Provides clinically relevant medication-related education to nursing staff.

10. Serves on the Medicine/Infection Control/P&T Committee.

11. Assists in tracking, analyzing, and presenting hospital antibiogram information. Promotes antimicrobial stewardship throughout the facility.

12. Participates in community outreach projects including drug takeback and health fairs.

13. Tracks the need for compounded sterile products throughout the facility. Mixes sterile products or checks products mixed by a pharmacy technician.

14. Double check drugs before they are loaded into the Omnicell.

15. Assists with billing of medications in the electronic health record software as required.

16. Identifies, researches, and provides strategies for managing drug shortages.

17. Detects, monitors, documents, and reports adverse drug reactions and medication errors.

18. Maintains equipment databases including IV pumps, medication dispensing cabinets, and electronic health record software.

19. Assists pharmacy manager with responsibilities in accordance with the organization's mission, policy, and applicable laws

20. Actively participate in performance improvement and continuous quality improvement activities.

21. Develops policies and processes to assure compliance with standards of practice from accreditation organizations including The Joint Commission, and CMS.

22. Performs other related duties as assigned or requested.

Job Qualifications:

Education:

Required: Bachelor of Science degree in pharmacy

Preferred: Doctor of Pharmacy degree

Experience:

Required: None

Preferred: One year experience in a hospital setting

License/Certifications:

Required: Valid Nebraska Pharmacist license or the ability to become a licensed pharmacist in Nebraska within 90 days of hire
