Purpose: The Pediatric CRNP will provide comprehensive primary care to pediatric patients and their families while working collaboratively with physicians and the multidisciplinary Cahaba team. Candidates with a Pediatric Primary Care Nurse Practitioner (PPCNP) or Certified Pediatric Nurse Practitioner - Primary Care (CPNP-PC) background are encouraged to apply. A Family Practice CRNP with extensive pediatric experience will also be considered.
Responsibilities and Duties:
Provides comprehensive primary care to pediatric patients in accordance with established medical protocols, clinical guidelines, and the provider's scope of practice.
Evaluates pediatric patients through complete and interval histories, physical examinations, developmental assessments, and appropriate screenings.
Identifies patient risk factors and health hazards and provides appropriate health education to patients and parents/guardians.
Diagnoses and treats acute and chronic pediatric conditions within the provider's scope of practice.
Performs appropriate physical examinations as indicated.
Obtains appropriate laboratory specimens and analyzes clinical data to assist with diagnosis and treatment.
Develops and implements individualized plans of care based on the patient's clinical needs, age, developmental stage, family circumstances, and available resources.
Assures treatment and plans of care are consistent with established protocols and revises treatment as necessary based on patient outcomes.
Provides preventive healthcare services, including well-child visits, developmental screening, immunizations, and anticipatory guidance.
Provides education to patients and families regarding nutrition, development, medications, treatment plans, preventive care, and healthy lifestyle choices.
Documents all pertinent patient information accurately, legibly, and in a timely manner.
Assures appropriate consent forms are completed and maintained.
Refers patients to appropriate services when needs exceed the services of the clinic or the scope of practice of the provider.
Assists in assuring adequate patient health records are maintained and transferred as required when patients are referred.
Seeks out and utilizes community resources to meet or assist with the patient's healthcare and social needs.
Identifies and considers cultural patterns, economic status, family circumstances, and available patient resources when developing plans of care.
Collaborates with physicians, nurses, medical assistants, behavioral health providers, dental providers, and other members of the Cahaba care team to coordinate patient care.
Collaborates with physician preceptors and clinical leadership regarding patient care, protocols, and clinical concerns.
Participates in the development, execution, and periodic review of written policies governing the services provided by the clinic.
Provides services in accordance with Cahaba Medical Care policies, procedures, clinical protocols, and applicable regulatory requirements.
Participates in studies, audits, surveys, and other quality improvement activities as requested.
Participates in quality improvement activities as assigned by the Quality Improvement Officer (QIO) or Chief Medical Officer (CMO).
Participates in Cahaba's teaching mission and hosts learners as appropriate after completing the provider's initial orientation and integration period.
Provides appropriate clinical supervision and teaching to learners assigned to the provider's clinic.
Assists with daily lab, imaging, and medical record review for providers as assigned.
Participates in managing clinical registries and other population health initiatives as assigned.
Assists with Centering Group Visit Programs and other group-based care initiatives as applicable.
Participates in team meetings, provider meetings, and other organizational activities as required.
Performs other duties and responsibilities as assigned by the Chief Medical Officer or clinical leadership.
Qualifications:

Submits to the necessary components required to maintain privileging within the clinic, including background checks, drug screening, peer review by the CMO, certification of fitness to perform duties, and participation in applicable quality improvement programs.
