Under the direct supervision of the PACE Director of Center Operations, the Home Care Supervisor RN is responsible for coordination, supervision, and delivery of home care services for PACE participants. Supervises staff in their daily activities to ensure the delivery of care, ensuring compliance with regulatory and state guidelines. Supervision of Personal Care Assistants (PCA) and Licensed Vocational Nurses (LVN) delivery assistance with participant's activities in the center and home environments.
Location: Modesto County and Merced County.
Schedule: Monday - Friday, 8am - 5pm
Compensation: $55.28 - $64.64 per hour

Duties and Responsibilities - Responsible for managing all home care employees, vendor schedules and finding coverage for vendor or employee call outs.
- Responsible for timely reconciliation of vendor invoices and approving them for payment.
- Communicates participant changes with team members.
- Coordinates 24-hour home care services, including nursing, PT, OT, ST, personal care, DME, and contracted services, to ensure participant needs and Plan of Care goals are met.
- Supervises home care staff and contracted services, including staffing, scheduling, coverage, resource management, performance, evaluations, service quality, and corrective action in collaboration with the Director of Center Operations.
- Provide after-hours support for clients, referral partners and prospective clients - this support may include e-mail, telephone and/or home visits.
- Assists in developing policies and procedures as they relate to Home Care Services.
- Coordinates orientation and onboarding of new home care staff and provides ongoing coaching, education, and operational support.
- Serves as the PACE Home Care Coordinator, overseeing required assessments, reassessments, care planning, IDT participation, documentation, and home care regulatory requirements.
- Monitors home care service delivery and documentation, identifies quality or compliance gaps, and implements corrective actions as needed.
- Supports internal and external regulatory audits, surveys, and monitoring activities related to PACE home care services.
- Partners with clinical informatics and operational teams on EMR upgrades, testing, implementation, training, and optimization that impact home care workflows.
- Evaluates home care processes and recommends workflow and technology improvements to improve efficiency, documentation, compliance, and quality of care.
- Provides ongoing staff education related to EMR workflows, documentation standards, regulatory requirements, new functionality, and operational process changes. Manages staff performance and employee relations activities, including disciplinary actions, performance reviews, scheduling, payroll reconciliation, budgetary practices, performance improvement activities, and off-hours administrative or clinical support. Participates in IDT, staff, and committee meetings and communicates participant changes to appropriate team members. Observes for changes in participants' physical, mental, emotional, or social functioning and ensures appropriate communication and follow-up. Other projects and duties as assigned.
Physical Demands - Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate.
- Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
- Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
- Must have high manual dexterity.
- Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.
Work Environment - Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
- Subject to unpleasant odors.
- The noise level is usually quiet to moderate, but may at times be noisy and crowded.
Education/Experience Requirements Minimum Qualifications- Valid CA Driver's License, acceptable driving record, and vehicle insurance.
- Must be able to work effectively in a team environment.
- Must be able to treat geriatric patients.
- Only act within the scope of his or her authority to practice.
- Ability to communicate skillfully, handle multiple priorities, maintain a warm and friendly demeanor, perform nursing duties, be flexible, innovative, focus on results and travel regularly.
- Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
- Meet a standardized set of competencies established by Central Valley PACE and approved by CMS before working independently.
Education/Experience - Current BLS CPR Card certified by American Heart Association or American Red Cross.
- Minimum two (2) years supervisory or management experience in a health care setting.
- Minimum two (2) years' experience working with the frail or elderly population.
- Current unencumbered CA Registered Nurse (RN) License.
- Minimum three (3) years of registered nursing experience within the last five (5) years in a home health agency, primary care clinic or health facility.
- A bachelor's degree or higher degree in nursing or other health related field.
