Pay Range: $78,660 - 90,459 /Yr.

Schedule: Monday - Friday; 8:00 am - 4:30 pm

Job Summary

The Occupational Therapist will work with clients to build individual rehabilitation plans designed to help them develop or regain physical or mental functioning through occupational-based activities in the client's natural setting; i.e. homemaking skills, activities of daily living, sensorimotor, educational, recreational, vocational, and social activities. The Occupational Therapist will provide community based services to clients in order assess skills and to support maintenance of skills and safety in the community. They will provide and ensure the team provides client-centered, strengths-based, and trauma-informed services to clients with severe mental illness and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach.

Responsibilities
  • Assess and evaluate client care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment using standardized procedures
  • Assess client's personal, medical, social, emotional, and environmental situation to plan for linkage and treatment course. Develop and implement individualized treatment plans based on client needs containing achievable functional goals.
  • Attend meetings to consult with teams regarding client progress, behavior, and response to treatment
  • Collaborate with interdisciplinary professionals; i.e. Primary Care Physicians, Psychiatrists, Registered Nurses, Case Managers, Therapists, etc. to provide comprehensive care to support the client's successful independent living
  • Comply with evaluation, treatment, and documentation guidelines; and consistently maintain timely and accurate departmental records
  • Perform environmental evaluations and suggest modifications to increase safety and independence in the home as needed
  • Provide direct supervision to Level I and Level II Occupational Therapy students as assigned.
  • Perform other related duties and/or projects as assigned
  • Complete daily progress notes based on services provided to clients in a timely manner reflected in Trilogy's documentation policy and agency standards.
  • Engage regularly in team meetings to consult and collaborate on caseloads.

Qualifications
  • Master's Degree or Doctorate Degree in Occupational Therapy required
  • Licensed Occupational Therapist in the State of Illinois required
  • Qualified Mental Health Professional that must have one of the following requirements:
    • Licensed Practitioner in the Healing Arts;
    • Registered Nurse under licensed under the Illinois Nurse Practice Act;
    • Occupational Therapist licensed under the Illinois Occupational Therapy Practice Act [225 ILCS 75] with one-year experience in a mental health setting;
    • Master's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field
  • Valid Illinois driver's license with daily access to a well-maintained vehicle with $1,000,000/$3,000,000 liability insurance
  • Minimum one year experience working with people with mental health and/or substance abuse diagnoses
  • Experience in community-based services highly preferred
