Occupational Therapist (OT) - Home HealthServing patients across Pueblo and surrounding communities | [Full-Time]
Compensation: $130,000- $150,000 annual salary
Schedule: Full-time, Monday through Friday

What You'll DoAs a Home Health OT, you will deliver personalized therapy that helps patients participate more safely and independently in daily life:
- Evaluate functional abilities, routines, safety risks, and barriers within the home
- Develop and update individualized therapy plans aligned with physician orders and patient goals
- Provide interventions that improve strength, coordination, endurance, cognition, and daily living skills
- Teach adaptive techniques for bathing, dressing, grooming, meal preparation, and other activities
- Recommend adaptive equipment and environmental modifications that support safety and independence
- Educate patients and caregivers on exercises, strategies, equipment, and the plan of care
- Track progress, adjust interventions, and maintain timely, accurate documentation
- Collaborate with nursing, physical therapy, speech therapy, and other care team members
What You Bring- Graduate of an accredited occupational therapy program with an active Colorado OT license
- Minimum of one (1) year of occupational therapy experience preferred
- Home health experience preferred
- Current CPR certification
- Strong clinical judgment, creativity, communication, organization, and documentation skills
