Full Job Description
Shape the Future of Cancer Care
WMCHealth's Cancer Center, in collaboration with the Center for Advanced Practice, is seeking a Nurse Practitioner (NP) for ambulatory hematology/oncology and infusion services. The NP will provide comprehensive outpatient assessment, treatment-related follow-up, symptom management, and infusion support in partnership with oncologists, NPs, PAs, infusion nurses, pharmacists, and other interdisciplinary team members. This position will cross-cover Health Alliance Hospital in Kingston, NY, and MidHudson Regional Hospital in Poughkeepsie, NY.
Ambulatory Clinic and Infusion Responsibilities:
Conduct comprehensive and focused outpatient histories and examinations; assess functional status, comorbidities, medication use, symptoms, and treatment toxicities; order and interpret laboratory and diagnostic studies within scope and privileges.
Provide oncology clinic visits, pretreatment assessments, on-treatment follow-up, and post-treatment surveillance. Apply disease biology, staging, and relevant biomarker information to care planning with the oncology team.
Assess readiness for chemotherapy, immunotherapy, targeted therapy, and supportive infusions. Review the treatment plan, laboratory results, organ function, medication interactions, and documented consent; prescribe, renew, hold, or modify therapy only within granted privileges and approved treatment protocols, with oncology consultation as indicated.
Provide clinical support in the infusion center with infusion nurses and pharmacy. Participate in required treatment-order verification and safety checks; follow hazardous-drug, infection-prevention, and medication-safety policies.
Evaluate and manage cancer-related symptoms and treatment adverse effects using current evidence-based pathways. Recognize infusion reactions, extravasation, immune-related adverse events, and other oncologic emergencies; initiate site-approved emergency measures and arrange urgent oncology consultation or emergency-department transfer when care exceeds ambulatory capabilities.
Provide telephone and electronic symptom triage during assigned coverage, timely reassessment, and closed-loop follow-up of abnormal results. Monitor oral anticancer medication adherence, access, interactions, and toxicity with nursing and pharmacy partners.
Educate patients and caregivers using shared decision-making and teach-back about treatment goals, expected adverse effects, self-management, medication safety, and when and how to seek urgent help. Address fertility preservation, pregnancy prevention, and reproductive risks when relevant.
Coordinate supportive and palliative care, survivorship, prevention, psychosocial support, and referrals. Assess language, cultural, transportation, financial, and other barriers to care and connect patients with appropriate resources.
Cross-cover ambulatory clinic and infusion services at Kingston and Poughkeepsie. Maintain required credentialing and site-specific orientation at both locations; use consistent clinical pathways and structured handoffs that identify active treatment, pending results, toxicity concerns, follow-up responsibility, and escalation contacts.
Document timely care plans in the electronic health record; use approved digital and telehealth tools when applicable. Participate in evidence appraisal, quality improvement, safety reporting, interdisciplinary learning, and ongoing oncology competency validation.
Why Join Us?
