Current State license as a Registered Nurse and Advanced Practice Nurse (APN)
Valid Driver's License and reliable transportation with current auto liability insurance

Responsibilities
Consult with and educate patients and families on disease processes and self-care techniques
Assist in developing patient/family care plans with the Interdisciplinary Group (IDG)
Collaborate with attending and consulting physicians on palliative care
Communicate changes in patient needs and evaluate responses to care
Submit all documentation in a timely manner
Perform additional duties as required
Ensure compliance with local, state, and federal regulations and established policies

Nurse Practitioner
What You Must Have:
Graduate of a Baccalaureate School of Nursing
Current State license as a Registered Nurse and as an Advanced Practice Nurse (APN)
Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
Two years of experience as an APN in a clinical care setting, hospice or home health preferred
Comprehensive knowledge of pharmacology, palliative care, and end-of-life
What You Will Do:
Consult with and educate the patient/family regarding disease process, self-care techniques, end-of-life care
Consult with and assists in the IDG's development of patient/family plan of care to assure quality care is provided
Interact with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physician
Inform physicians and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care
Timely submission of all documentation
Performance of other duties as required
Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
