Job Summary

We are seeking an experienced and compassionate Nurse Practitioner (NP) to join our growing team in a hybrid role providing both Primary Care and Urgent Care services. This position offers the opportunity to deliver comprehensive, patient-centered care across the continuum, managing chronic conditions while also evaluating and treating acute illnesses and injuries.

The ideal candidate is comfortable working in a fast-paced environment, demonstrates excellent clinical judgment, and is committed to providing high-quality, accessible healthcare to a diverse patient population.

Responsibilities Primary Care
  • Provide comprehensive primary care services for pediatric (if applicable), adult, and geriatric patients.
  • Perform annual wellness visits, preventive screenings, and routine physical examinations.
  • Diagnose, treat, and manage acute and chronic medical conditions.
  • Develop individualized treatment plans and provide patient education.
  • Order, interpret, and communicate laboratory and diagnostic test results.
  • Prescribe medications and monitor treatment effectiveness.
  • Coordinate specialty referrals and follow-up care.
Urgent Care
  • Evaluate and treat patients presenting with acute illnesses and minor injuries.
  • Diagnose and manage conditions including respiratory infections, influenza, COVID-19, urinary tract infections, skin conditions, gastrointestinal complaints, musculoskeletal injuries, and other urgent medical concerns.
  • Perform minor office procedures including wound care, laceration repair, incision and drainage, splinting, foreign body removal, and abscess management (within scope of practice).
  • Interpret basic diagnostic testing including EKGs, point-of-care testing, and imaging results as appropriate.
  • Determine appropriate disposition, including emergency department referrals when indicated.
Clinical Responsibilities
  • Maintain accurate and timely documentation in the electronic medical record (eClinicalWorks preferred).
  • Collaborate with physicians, specialists, nursing staff, and care management teams to ensure coordinated patient care.
  • Participate in quality improvement initiatives and population health programs.
  • Ensure compliance with organizational policies, evidence-based clinical guidelines, and regulatory requirements.
  • Educate patients and families on disease prevention, wellness, medications, and treatment plans.
Qualifications Required
  • Master's or Doctorate degree from an accredited Nurse Practitioner program.
  • Current New York State Nurse Practitioner license.
  • National Board Certification (AANP or ANCC).
  • Current DEA registration and New York State Controlled Substance Registration.
  • Minimum of 2 years of Nurse Practitioner experience in Primary Care, Family Medicine, Internal Medicine, Urgent Care, or Emergency Medicine.
  • Current BLS certification.
  • Strong clinical assessment, diagnostic, and procedural skills.
  • Excellent communication and patient education abilities.
Preferred
  • Experience with eClinicalWorks (eCW).
  • Bilingual (Spanish or another language) preferred.
  • Experience treating patients across all age groups.
  • ACLS certification.
