Responsibilities
Act as the lead provider of internal medicine services in the Skilled Nursing Facility
Collaborate with interdisciplinary teams and physicians to oversee resident care
Manage day-to-day care of both short and long-term residents with nursing staff support
Conduct evaluations for new admissions
Coordinate discharge visits and plan follow-up care
Perform monthly visits for long-term residents to monitor stability
Assess patients for changes in condition and provide necessary interventions
Prescribe treatments and manage acute/chronic diseases with established protocols

This position is focused in a Skilled Nursing Facility (SNF) setting, where you would serve as a clinical leader, working collaboratively with the facility's care team to oversee both short- and long-term residents. Responsibilities include admissions, discharges, monthly stability checks, and evaluations for changes in condition. You'll also handle advanced care planning, comprehensive assessments, and prescribing treatments.
