Position Summary
Nurse Practitioner or Physician Assistant to provide high-quality care for long-term care residents by assessing, diagnosing, treating, and coordinating care alongside physicians and the interdisciplinary team.
Key Responsibilities
Conduct adult/geriatric assessments and physical exams
Manage acute and chronic health conditions
Collaborate with physicians, families, and care teams
Support care transitions and treatment planning
Participate in quality improvement initiatives
Qualifications:
Qualifications
Active NP or PA license in the state of practice
Board certification (AANP, ANCC, or equivalent)
Minimum 2 years of clinical experience
Active DEA license
Current NPI and Medicare provider enrollment
CPR certification
