The Role

We are seeking a compassionate and clinically confident Nurse Practitioner (NP) or Physician Assistant (PA) to provide high-quality primary care services to geriatric patients in skilled nursing and post-acute care settings throughout Crown Point and St. John, Indiana.

In this role, you will work collaboratively with an experienced physician while maintaining a high level of autonomy as you manage a panel of patients across multiple facilities. Responsibilities include:

  • Conduct comprehensive primary care visits for geriatric patients in Skilled Nursing Facilities (SNFs) and other post-acute care settings.
  • Diagnose, treat, and manage acute and chronic medical conditions while promoting preventive care and improving overall patient outcomes.
  • Perform routine follow-up visits, evaluate changes in patient condition, and develop individualized treatment plans.
  • Review and reconcile medications to ensure safe and effective therapy while minimizing polypharmacy risks.
  • Order, interpret, and follow up on laboratory tests and diagnostic studies.
  • Coordinate care with facility staff, specialists, therapists, and family members to ensure seamless communication and continuity of care.
  • Accurately document patient encounters and maintain complete, timely medical records.
  • Maintain independent clinical decision-making while collaborating closely with supervising physicians and the interdisciplinary care team.
