Our Advanced Practice Providers, (Nurse Practitioners and Physician Assistants), provide both acute and chronic care management for adult and geriatric patients in our
Janesville, Beloit, Edgerton and Monroeservice areas. Advanced Practice Providers collaborate with supervising physicians to develop and implement comprehensive treatment plans that improve patient outcomes and quality of life.
Nurse Practitioner/Physician Assistant Key Responsibilities- Provides comprehensive primary care to adult and geriatric patients, managing a broad range of acute and chronic medical conditions.
- Develops and implements individualized treatment plans in collaboration with the facility care team, patients, families, and other healthcare providers.
- Performs comprehensive patient assessments; establishes medical diagnoses; prescribes medications; and performs diagnostic and therapeutic procedures, as clinically appropriate.
- Ensures timely, accurate, and thorough documentation of patient assessments, diagnoses, treatment plans, interventions, and follow-up care.
- Collaborates closely with specialty providers, including wound care, psychiatry, and other disciplines, to facilitate coordinated, integrated, and patient-centered care.
- Communicates changes in patient condition, clinical findings, and treatment plans effectively with the interdisciplinary care team, patients, families, and facility staff to promote understanding and continuity of care.
- Cultivates strong, collaborative relationships with patients, families, facility staff, and healthcare providers to promote high-quality outcomes and continuity of care.
- Performs other duties and responsibilities as assigned.
Nurse Practitioner/Physician Assistant - Required Qualifications- Current, unrestricted licensure as a Nurse Practitioner or Physician Assistant.
- Current, valid DEA registration, or eligibility to obtain DEA registration.
- Current Basic Life Support (BLS) certification.
- Demonstrated clinical expertise and strong diagnostic and critical-thinking skills.
- Ability to exercise sound clinical judgment and work effectively with appropriate autonomy.
- Demonstrated ability to collaborate effectively within interdisciplinary care teams.
