Full Job Description
We are seeking a compassionate and dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our outpatient primary care team. This is an excellent opportunity for a provider who is passionate about delivering high-quality, patient-centered care and improving access to helathcare within the community.

The position is 100% outpatient. You will work as part of a collaborative, multidisciplinary team focused on providing comprehensive primary and preventive care to a diverse patient population.

Key Responsibilities:
  • Provide comprehensive outpatient primary care to patients across the lifespan, as appropriate to training and experience
  • Evaluate, diagnose, and manage acute and chronic medical conditions
  • Perform routine physical examinations and preventive health screenings
  • Develop and implement individualized treatment plans
  • Order and interpret appropriate laboratory and diagnostic studies
  • Prescribe medications and treatments within scope of practice
  • Educate patients regarding preventive care, chronic disease management, medications, and healthy lifestyle choices
  • Coordinate care with Physicians, behavioral health professionals, MA staff, specialists and community resources
  • Maintain accurate and timely documentation in the electronic health record
  • Support quality improvement initiatives and FQHC clinical performance measures
  • Provie culturally responsive, compassionate care to underserved and medically vulnerable populations

Qualifications:
  • Graduate of an accredited Nurse Practitioner or Physician Assistant program
  • Current or eligible NP or PA licensure in Georgia
  • Current national board certification
  • DEA registration or eligibility to obtain
  • Current BLS certification
  • Strong clinical judgement, communication, and interpersonal skills
  • Ability to work effectively within a team-based care environment
  • Commitment to serving diverse and underserved patient populations
  • Primary care, Family medicine, internal medicine, or community health experience preferred
  • New graduates may be considered depending on training and clinical experience
