MVHC is growing and has an immediate opportunity for a Nurse Practitioner to join our Transition of Care/Primary Care Team!

Aspects of this Position:- 100% outpatient only
- Opportunity to work with a well-established team consisting of physicians, mid-level providers, licensed social workers, RNs and LPNs
- Close proximity to community hospital for easy access to additional services
- Epic EMR
- No Call
Minimum Qualifications:- Master of Science in Nursing
- NP Certification
- One to two years of clinical experience preferred
- CPR Certification
- Supervisory experience a plus
- DOT Certification is preferred
Key Responsibilities:- Conduct virtual Transition of Care visits within required post-discharge timeframes.
- Perform comprehensive post-hospital assessments and evaluate patients for ongoing medical needs.
- Complete medication reconciliation and address medication-related concerns.
- Review hospital records, discharge summaries, laboratory results, and diagnostic testing.
- Develop and implement individualized care plans.
- Identify and address barriers to recovery, including social determinants of health.
- Provide chronic disease management and primary care services when appropriate.
- Order laboratory tests, diagnostic studies, medications, and referrals as clinically indicated.
