Description:Mercy Care Atlanta is seeking a Nurse Practitioner (NP) to join our:
Street Medicine Team
Mercy Care's Street Medicine Team brings healthcare directly to individuals experiencing homelessness and others who face significant barriers to accessing traditional healthcare services. This role combines clinical practice, outreach, advocacy, and care coordination to address immediate health concerns while helping patients navigate a path toward long-term stability and ongoing healthcare services.

- Assess, diagnose, and treat acute and chronic medical conditions among individuals experiencing homelessness.

- Connect patients to housing resources, behavioral health services, specialty care, substance use treatment, and other supportive services.
- Develop individualized care plans designed to improve health outcomes and reduce barriers to care.
- Document patient encounters and coordinate follow-up care across Mercy Care clinics and community partners.
- Participate in outreach activities focused on engaging hard-to-reach populations and establishing continuity of care.
- Advocate for patients and serve as a trusted healthcare partner for individuals who may have had limited access to healthcare services.
