Active Nurse Practitioner license in the state of practice
Minimum 3 years of experience in outpatient care
Experience working with senior populations
Strong background in primary care and preventive medicine
Excellent communication and patient management skills
Responsibilities
Provide comprehensive primary care to senior patients
Conduct health assessments and develop treatment plans
Collaborate with multidisciplinary teams for holistic patient care
Educate patients and families about health maintenance and disease prevention
Manage medication dispensing and coordinate follow-up care

Job Description Well-respected health system seeks an experienced Nurse Practitioner. Practice is committed to helping seniors lead healthier, happier lives through superior medical care. In addition to primary care, the clinic also provides cardiology, imaging, nutritional services, medication dispensing & resource support on-site. This is full-time outpatient only w/no inpatient work required. Office is open only Mon-Fri 8am-5pm. Call coverage shared (only by phone).
