Job Summary:

The Nurse Practitioner shall be licensed in the State of Mississippi and must practice according to a Board approved protocol which has mutual agreement of and by a Mississippi licensed physician and the approval by the Board of Trustees. Performs under the supervision of the Executive Director Post Acute Care and Chief Medical Officer and in collaboration with the Medical Director of Palliative Care and the Executive Director of Post-Acute Care.

• Provides palliative care for patients focused on symptom management and conducts goals of care conversations to elicit patients/families understanding of their condition, treatment options, and goals of care.
• Confers with the Medical Director of Palliative Care regarding assigned patients.
• Keeps physicians and other caregivers informed of needs and problems related to patient care.
• Works with discharge planning entities to enhance coordination of palliative care post-discharge.
• Participates in weekly Interdisciplinary Team Meetings, Palliative Care Committee meetings, and others as assigned.
• Cooperates with other personnel to achieve the objectives of palliative care and maintain good employee/interdepartmental/and public relations.
• Represents a cohesive voice of the palliative care program in working with medical and hospital staff.
• Participates in and/or provides educational programs for hospital staff in palliative care.
• Maintains patient rights and confidentiality of patient information, and upholds the We CARE values.
• Demonstrates knowledge and skills to appropriately communicate and interact with the patients, families, and visitors of all age groups while being sensitive to their cultural and religious beliefs.
• Performs other job duties as applicable.

Qualifications:

Education/Skills

Graduate of a recognized non-online RN program required. Master's Degree in Nursing required.

Work Experience:

Previous work experience as a Nurse Practitioner preferred. Completion of at least 720 clinic hours required. Palliative Care, Gerontology, Acute Care, or Oncology experience preferred.

Certification/Licensure-DUE UPON HIRE

• Licensed as an APRN by Mississippi State Board of Nursing required.
• Certification as a Family Nurse Practitioner by the American Nurse Credentialing Center (ANCC) required.

Additional Certification/Licensure - Obtained based on required timeframe below

• Certification in Palliative Care-Advanced Certified Hospice and Palliative Nurse (ACHPN)
