As a Nurse Navigator at Schroeder Ambulatory Centre, you will champion a patient- and family-centred approach to delivering comprehensive, high-quality, and safe care. Guided by evidence-based nursing practice and SAC standards, you will work both independently and collaboratively within an interprofessional team to create an exceptional care experience for patients and their families.
The Nurse Navigator provides coordinated support throughout the surgical journey - from initial assessment and referral, through treatment, recovery, and post-operative follow-up. In this role, you will serve as a key point of contact, ensuring seamless communication, continuity of care, and timely access to services, contributing to improved patient outcomes and overall satisfaction.
Key ResponsibilitiesReferral Review & Patient Intake- Review surgical and procedural referrals to ensure appropriateness for care at SAC
- Apply established screening criteria to determine suitability for ambulatory procedures
- Coordinate with physicians, anesthesia, and clinical teams when additional review is required
- Contact patients to complete health history screening and pre-assessment Pre-Operative Coordination
Pre-Operative Coordination- Provide patients and families with clear education regarding their procedure, preparation, and expectations
- Coordinate any required pre-operative testing, consultations, and documentation
- Identify and address potential barriers to care prior to procedure date
- Ensure all required documentation, consents, and assessments are completed
Care Navigation & Communication- Act as the primary point of contact for patients throughout their surgical journey
- Facilitate communication between patients, surgeons, anesthesia providers, nursing staff, and administrative teams
- Ensure seamless information flow across departments and systems
- Support scheduling coordination to optimize patient flow and readiness
Day-of-Procedure Support- Support patient flow and readiness in collaboration with SDC, OR, and PACU teams
- Assist in resolving issues that may impact procedure flow or patient experience
- Provide reassurance and support to patients and families
Post-Operative Follow-Up- Conduct post-operative follow-up calls and assessments as appropriate
- Reinforce discharge instructions and identify early concerns or complications
- Coordinate additional follow-up care or referrals as required
Quality, Safety & Program Development- Promote adherence to SAC standards, IPAC practices, and patient safety initiatives
- Identify opportunities to improve patient experience and care processes
- Participate in quality improvement initiatives and workflow optimization
- Support the development and refinement of navigation processes as SAC programs expand
- Perform additional duties and undertake special projects as assigned
Corporate Responsibilities:- Participate in all mandatory training requirements and work in compliance with the Occupational Health and Safety Act and Regulations, Schroeder Ambulatory Centre Policies and Procedures, as well as established industry guidelines
- Be responsible for and ensure that all staff supervised are trained and work in compliance with the Occupational Health and Safety Act and Regulations, Schroeder Ambulatory Centre Policies and Procedures, as well as established industry guidelines
- Perform additional duties and undertake special projects as assigned
Education and Qualifications- Current registration in good standing with the College of Nurses of Ontario (CNO)
- Bachelor's degree in healthcare-related field (e.g., nursing, health sciences).
- Minimum 3-5 years of nursing experience in surgical, perioperative, or ambulatory care settings
- Strong knowledge of pre-operative and post-operative care requirements, clinical best practices and evidence-based research.
- Demonstrated experience in care coordination or patient navigation preferred
- Current BLS certification
- Excellent communication and interpersonal skills
- Strong organizational and critical thinking abilities
- Ability to work independently and prioritize in a fast-paced environment
- Patient advocacy and commitment to patient-centred care
- Collaborative team approach with strong problem-solving skills
- Comfort working across multiple systems and coordinating complex workflows
- Knowledge and Experience with Meditech is an asset
