Job DetailsOur Rehab Admissions Nurse Liaison (RN) will:
- Evaluate patients for admission to the St. Charles Inpatient Physical Rehabilitation Program
- Travel and visit with patients currently admitted at other local hospitals as well as on the St. Charles main campus, and assess for admission into the inpatient rehab program
- Educate patients and family members about inpatient rehab
- Determine medical stability and readiness of the patient for transfer into the rehab program
- Coordinate a smooth transition from the referring facility to the rehab setting
- Partner with an interdisciplinary team including, but not limited to, attending physicians, Registered Nurses (RNs), and ancillary staff to optimize patient outcomes and satisfaction.
- Advocate for and build meaningful relationships with patients and their loved ones through safety intervention, emotional support, and education.
What you'll bring:
- Passion for Rehab Nursing
- 5+ Years of clinical hospital based RN experience required
- Previous Subacute Rehab experience also preferred
- Education: Graduate of an accredited School of Nursing
- Bachelor of Science in Nursing (BSN) degree strongly preferred
- Licensure: New York Registered Nurse (RN) License & Registration
Posted Salary RangeUSD $56.24 - USD $68.00 /Hr.
