Are You Ready to Lead in Healthcare? We are currently seeking a Physical Therapist (PT) to join our outstanding healthcare team!

Salary Range: $58.00-$63.00 per hour - based on experience

If you're a compassionate Physical Therapist (PT) who is passionate about helping patients regain mobility and independence, we'd love to meet you. Join a team dedicated to delivering exceptional home health services while supporting one another through collaboration, respect and excellence.

As a Physical Therapist (PT), you will evaluate patients, develop individualized treatment plans, improve strength, balance, mobility, and functional independence, and educate patients and caregivers on safety and fall prevention. You will collaborate with physicians and the interdisciplinary team to maximize patient outcomes. You will make a meaningful impact by helping patients safely recover and remain independent in their own homes.

In this role, you will build meaningful relationships with patients and families while promoting independence, improving outcomes, and ensuring compassionate, patient-centered care.

Must Have Qualifications:
• Current California Physical Therapist license
• Current BLS certification
• Valid driver's license, reliable transportation, and auto insurance
• Strong clinical assessment and treatment planning skills
• Ability to work independently while collaborating with an interdisciplinary team
• OASIS documentation experience

Nice to Have Qualifications:
• Previous experience working in home health, hospice, or a similar caregiving
environment
• Familiarity with KanTime Electronic Medical Record (EMR) system
• Bilingual skills are a plus
