Job Description:
Summary:
The Medication Safety Officer (MSO) is responsible for the development, organization, and coordination of the organization's overall medication safety program. This innovative leader will set the vision and direction to ensure medication safety best practices are utilized within Ballad Health. The MSO will serve as the authoritative expert in safe medication use and shall coordinate medication safety assessments to identify opportunities to improve the medication use system, educate others on the importance of medication error reporting, participate in the review and investigation of medication errors, and lead implementation of error-prevention strategies. They will communicate at the highest professional level with pharmacists, physicians, nurses and other professional staff and committees within the organization in order that they may effectively cultivate the trust of stakeholders, guide institutional thinking and behavior, and drive culture change.

Requirements:
Must be a graduate of an ACPE accredited college of pharmacy with an advanced pharmacy degree (e.g. Pharm.D.) and:PGY-2 in a relevant Pharmaceutical Specialty. orPGY-1 with relevant work experience.
Minimum of five (5) years of related experience, three (3) of which are in the inpatient setting of a large hospital, and two (2) years of which are in leading health and quality improvement initiatives in the patient/ medication safety field, including proactive risk assessments (e.g. failure mode and effects analysis) and adverse event investigations (e.g. root cause analysis).
Completion of an Institute for Safe Medication Practices (ISMP) Fellowship, Intensive or Mentorship, or completion of an ASHP Medication Safety Certificate Program or Certified Professional in Patient Safety (CPPS) preferred.
Prior multi-site or system level experience preferred.

Licenses and Certifications:
Must hold, or be eligible to hold a pharmacy license in the State of Tennessee and/or Virginia.Board of Pharmaceutical Specialties Certification in at least one discipline.
