Are You Ready to Lead in Healthcare? We are currently seeking a Full-Time Minimum Data Set Nurse (MDS) to join our outstanding healthcare team!

Salary Range: $95k-$100k for LVN. $104k-124k for RN. annually.

As the Full-Time Minimum Data Set Nurse (MDS), you will aid in conducting and coordinating the development, completion, and transmission of the resident assessment in accordance with state requirements and the policies and goals of the facility. Your role will ensure that the MDS process is carried out effectively, supporting the overall care of residents and facilitating compliance with necessary regulations.

Your key responsibilities include:
  • Assisting in the completion of the Minimum Data Set (MDS) assessment for all residents.
  • Coordinating the MDS process, ensuring accurate and timely submission in accordance with state and federal regulations.
  • Collaborating with interdisciplinary teams to gather necessary data and support the resident's care planning process.
  • Maintaining accuracy and integrity of resident information by ensuring all data is entered correctly.
  • Monitoring resident status and changes that may require updates to MDS assessments.
  • Assisting with compliance with the regulatory requirements governing MDS, including Medicare participation.

Must Have:
  • 1-3 years of MDS experience
  • Nursing degree from an accredited college
  • Current and unencumbered LVN/LPN license in the state
  • Knowledge of clinical standards of practice, regulations, and reimbursement governing long-term care and Medicare participation
  • Must be knowledgeable of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care

Nice to Have:
  • Experience providing care to multiple residents
  • Experience in a Skilled Nursing environment
  • Experience with PDPM (Patient-Driven Payment Model)
