WHAT YOU WILL DO AS A LONG-TERM AND POST-ACUTE CARE NURSE PRACTITIONER:- Provide direct care for patients with chronic and acute conditions
- Collaborate with physicians to guide treatment plans
- Partner with facility leadership and interdisciplinary teams
- Help support MedCap Health's rollout into a new community
Qualifications:- 1+ years of experience in post-acute/SNF/LTC
- Board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
- Post-acute, long-term, and/or skilled nursing facility experience

WHAT WE OFFER OUR TEAM:Pay: This
contract role offers a highly competitive annual range of
$150,000 to $200,000, rewarding your expertise and impact.
