Licensed Practical Nurse (LPN) – Home Health
Full-Time | Boston Territory Monday–Friday, 8 a.m.–5 p.m. | $38-$42/hour, based on experience

A great nursing visit can change someone’s entire day. It might mean recognizing a concern early, helping a caregiver feel more confident, or supporting a patient as they regain independence.
At Elara Caring, we’re seeking a Licensed Practical Nurse to bring compassionate, skilled care to patients throughout Boston. You’ll build meaningful relationships through one-on-one visits and work closely with an RN Case Manager and clinical team to support each patient’s care.
What You’ll Do
Provide skilled nursing care according to each patient’s individualized care plan and physician orders.
Monitor patient progress and promptly report changes in condition or safety concerns to the RN Case Manager.
Teach patients and caregivers self-care techniques that support health, safety, and independence.
Collaborate with the RN Case Manager as patient needs and care plans change.
Document visits, nursing interventions, and medication information accurately and promptly.
Communicate with physicians, families, and the clinical team to keep care coordinated.
Help reduce avoidable hospitalizations through attentive care and early recognition of concerns.
What You’ll Bring
An active Massachusetts LPN license
Graduation from an accredited nursing program
At least one year of clinical nursing experience
A valid driver’s license, reliable transportation, and current auto insurance
Ability to travel throughout the Boston territory
Ability to sit, stand, bend, lift, and move intermittently, including lifting 50–100 lbs.
