ResponsibilitiesResponsibilities include:- Assess nature of clients' situations by interviewing and reviewing personal history
- Develop and execute individual treatment plans
- Determine appropriate milestones to gauge client progress Communicate client updates to all relevant parties
- Facilitate referrals to other healthcare professionals and programs Maintain accurate client documentation
- crisis assessments/triage with appropriate interventions

- other related activities designed to provide integrated health care services
QualificationsJob Requirements:- Master's
- LCSW (New York license)
- one to two years' experience providing clinical services in the mental health field
- Bilingual English/Spanish a plus
- Hybrid
Salary: $80,000-$90,000
