Join Our Team: Nurse PractitionerResponsibilities:- Conducts face-to-face encounters for patients in their third or subsequent benefit period, documenting and sharing findings with certifying physician promptly.
- Conducts well-patient checks.
- Conducts symptom management visits.
- Provides physician support.
- Coordinates and collaborates with IDT members, community physicians, facility staff, and others as needed.
- Participates in developing Plan of Care for assigned patients.
- Assists in enhancing care strategies with Hospice Clinical Manager and Medical Director.
- Acts as a resource for Hospice staff to ensure continuity and improve patient care quality.
- Attends IDT meetings.
- Provides backup support to Hospice Medical Director, covering medical needs of hospice patients as required

Requirements:- Graduate of an approved Nurse Practitioner curriculum
- Active APRN or NP license required
- Minimum of two years clinical nursing experience
- Valid driver's license
