Qualifications

• Current RN license in the state of practice; associate degree preferred.
• Current CPR certification required (Georgia and California), preferred in other locations.
• Minimum one year of supervisory experience in healthcare.
• One year of nursing experience in hospice, home health, med-surg, or specific patient populations (e.g., geriatrics, pediatrics) required in Connecticut.
• Valid driver's license, reliable vehicle, and state-required liability auto insurance.

Responsibilities

• Coordinate daily clinical operations by assigning patient care responsibilities and managing admissions and transfers.
• Support staff performance through coaching, chart audits, and medication management oversight.
• Maintain communication with the Team Director, physicians, and interdisciplinary team to align care plans.
• Ensure patient and caregiver education is provided regarding medication use and end-of-life preparation.
• Oversee accurate and timely patient record-keeping, ensuring documentation is completed each shift.
• Participate in meetings, performance reviews, staff training, and stay current with hospice regulations.
