As a Home Visitor RN, you will:- Conduct in-home visits to provide nursing services aligned with the NFP model and Visit-to-Visit guidelines, managing a caseload of up to 25 clients
- Assess and address the health and developmental needs of women and their families
- Offer counseling and instruction to help families achieve targeted health and life goals
- Build and maintain strong community relationships to facilitate client referrals and resource connections
- Attend weekly team meetings, case conferences, and clinical supervision sessions
- Mentor and precept new hires and support student nurses during home visits as needed
- Ensure compliance with agency infection control and safety policies
- Maintain professionalism in adherence to organizational policies on absenteeism and appearance
Qualifications- Associate's degree in Nursing required
- 2-3 years of relevant nursing experience; home visiting or maternal/child health experience preferred
- Proficiency in computer use, excellent verbal and written communication, strong organizational and interpersonal abilities
- Bilingual (English and Spanish-speaking) preferred
- A commitment to CFHI's mission and the ability to build trusting relationships with clients and colleagues
This position is covered by a collective bargaining agreement and is represented by a Union.
In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $42.00 to USD $50.00.
