Hiring Range: $76,024.00 - $123,468.80 annually
Our Home Health Rehabilitation team delivers skilled physical, occupational, and speech therapy to adult and geriatric patients in their homes following illness, injury, or hospitalization. With a strong focus on post-acute recovery, fall prevention, mobility and ADL retraining, and patient/caregiver education, our Home Health team utilizes individualized, evidence-based care plans to promote independence, prevent readmissions, and support safe transitions across the continuum of care.
As a Physical Therapist, you will be responsible for examining, evaluating and testing individuals with mechanical, physiological and developmental impairments, functional limitations, and disabilities or other health and movement-related conditions in order to determine a diagnosis, prognosis and plan of treatment intervention, and assess the ongoing effects of intervention. The PT is also responsible for alleviating impairments, functional limitations and disabilities by designing, implementing and modifying treatment interventions. Additional responsibilities include reducing the risk of injury, impairment, functional limitation and disability, as well as, the promotion and maintenance of fitness, health and wellness in populations of all ages. The PT is engaged in administration, consultation, education and research. The PT is expected to conduct themselves in an effective professional behavior manner.
