What You'll Be Doing- Help patients restore function, improve mobility, reduce pain, and prevent disability
- Support patients in achieving the highest possible level of physical independence
- Participate in interdisciplinary care planning with the full home care team
- Perform comprehensive initial evaluations, OASIS Start of Cares (SOCs), and develop individualized plans of care
- Administer prescribed physical therapy treatments and skilled interventions
- Conduct 30-day reassessments, adjust care plans, and update documentation as needed
- Provide instruction to the patient and family on safe mobility, strengthening, and home programs
- Supervise up to two Physical Therapist Assistants (PTAs) in accordance with Nebraska state guidelines and agency policies
What We're Looking For- A genuine passion for helping others live healthier, more independent lives
- Graduate of an accredited Physical Therapy program
- Current Nebraska Physical Therapy license in good standing
- Current or eligible NPTE certification
- At least one year of experience in an acute care, rehab, or outpatient setting
- Home health experience (2+ years preferred)
- Strong clinical judgment and the ability to work independently
- Reliable transportation to visit patients in their homes
