The Admissions RN is responsible for conducting initial patient assessments, developing care plans, and ensuring smooth onboarding for patients entering home health services.
This role offers a 32- 40 hour work week with flexible AM start time to support work/life balance
$50 - $53 /hour
RESPONSIBILITIES:
Perform comprehensive in-home assessments, including physical, psychosocial, and environmental evaluations, to determine eligibility for home health services.
Establish individualized nursing care plans based on patient needs, physician orders, and regulatory requirements.
Provide education on disease processes, medication management, safety precautions, and self-care techniques.
Communicate with physicians, therapists, social workers, and other healthcare professionals to ensure a seamless transition to home health care.
Complete and submit all required documentation, including OASIS assessments, nursing notes, and care plans, in compliance with agency policies and regulatory standards.
Adhere to state and federal regulations, including Medicare and Medicaid guidelines, as well as agency protocols.
Identify potential risks, assess home safety, and ensure that patients receive appropriate follow-up care.
QUALIFICATIONS:
Education: Graduate of an accredited nursing program (RN required).
Licensure: Active RN license in the state of practice.
Experience: Minimum of 1-2 years of experience in home health, acute care, or related field preferred.
Certifications: BLS/CPR certification required; OASIS training preferred.
