OverviewThe Chief Clinical Officer is responsible for translating HarmonyCares’ clinical strategy into a consistent, scalable home-based care model across markets. This physician leader designs practical workflows, team roles, education, quality improvement routines, and program supports that help interdisciplinary teams deliver high-quality, patient-centered care. While the Enterprise Chief Medical Officer retains ultimate enterprise medical governance, the Chief Clinical Officer owns care model design, implementation, and continuous improvement in close partnership with the SVP of Population Health, field clinical leaders, operations, quality, product, and People & Culture.
Essential Duties and Responsibilities
- Design the home-based care model, including team roles, visit cadence, care planning, handoffs, escalation, and longitudinal accountability
- Translate evidence into practical pathways for frailty, serious illness, transitions, medication risk, behavioral health, and caregiver needs
- Define which standards are required enterprise-wide and where local adaptation is appropriate
- Build workable routines for high-risk review, panel management, interdisciplinary care, after-hours response, and closed-loop follow-up
- Lead the clinical design of population health programs with the VP/Senior Medical Director and SVP Population Health; test feasibility and impact before scale
- Partner with Quality and field medical leaders on Stars and HEDIS performance, risk adjustment documentation integrity, safety, and the clinical drivers of utilization
- Set interdisciplinary practice expectations, competencies, onboarding, clinical education, and professional support
- Partner with People & Culture and field leaders on workforce planning, top-of-license practice, clinician retention, and performance improvement
- Use case review, measurement, and field feedback to identify variation and remove unnecessary workflow burden
- Co-design clinical workflows and decision support with Product and Technology, with front-line clinicians involved in testing
- Set adoption and benefit expectations for clinical tools, including automation and AI
- Require patient safety safeguards, human oversight, and clear criteria to change or stop tools that do not work

Qualifications
Required Knowledge, Skills, and Experience
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) with active, unrestricted license
- Current board certification in Family Medicine, Internal Medicine, or Geriatric Medicine through a member board of the American Board of Medical Specialties (ABMS) or the American Osteopathic Association (AOA)
- 10+ years of senior leadership experience building and operating interdisciplinary care models across a scaled, distributed delivery organization
- Prior experience converting evidence and strategy into workflows, roles, tools, education, and measurable outcomes
- Demonstrated expertise in clinical operations, quality improvement, workforce practice, change management, and implementation
- Strong understanding of complex Medicare populations and value-based care economics
- Credibility with physicians and non-physician clinicians and a track record of leading across professional boundaries
- Home-based care, complex primary care, geriatrics, palliative care, community health, or longitudinal care-management experience preferred
- Formal training in quality improvement, implementation science, clinical informatics, public health, or healthcare administration preferred

- Prior experience with technology-enabled care delivery and clinical product development
