Family Nurse Practitioner (FNP)
Nexus Neurorecovery Center - San Antonio, TX Setting: Residential Treatment Center for Adolescent Females
We are seeking a Family Nurse Practitioner (FNP) who is comfortable caring for adolescents with behavioral health needs and collaborating with psychiatric and therapeutic providers in a residential treatment setting.
Position Summary
The Family Nurse Practitioner provides general medical care, health assessments, and management of acute and chronic medical conditions for adolescent female residents. Working under the direction of the Medical Director, the FNP helps ensure residents' medical needs are addressed consistently throughout their stay.
This position requires a provider who can build rapport with adolescents, use a calm and compassionate approach during challenging interactions, and recognize how behavioral health concerns may affect medical care. The FNP works closely with psychiatric providers, nurses, therapists, and direct care staff to support coordinated treatment.
Key Responsibilities
Medical Assessment & Treatment
Obtain detailed health histories and perform comprehensive physical examinations, including neurological assessments when appropriate.
Assess, diagnose, and treat acute and chronic medical conditions within the scope of FNP practice and facility privileges.
Address routine health concerns, preventive care needs, and ongoing medical issues throughout residential treatment.
Order appropriate laboratory studies and diagnostic procedures, interpret results, and initiate treatment or referrals as indicated.
Develop individualized medical care plans and monitor residents' response to treatment.
Recognize changes in medical condition and consult the Medical Director or attending physician when further evaluation is needed.
Care in a Behavioral Health Setting
Provide medical care to adolescent females with psychiatric diagnoses, emotional dysregulation, and challenging behaviors.
Use developmentally appropriate, trauma-informed communication to help residents feel safe and participate in their care.
Collaborate with psychiatric providers regarding medical concerns, medication-related symptoms, and changes in resident functioning.
Recognize behavioral or psychiatric concerns that require assessment by the appropriate treatment team member.
Support staff in distinguishing medical symptoms from behavioral presentations and coordinating appropriate follow-up.
Participate in required behavioral management and de-escalation training.
Medication Management & Continuity of Care
Prescribe and manage medications within authorized scope of practice, prescriptive authority, and facility guidelines.
Monitor treatment effectiveness, side effects, interactions, and required laboratory results.
Coordinate with psychiatric providers regarding psychiatric medication management and related medical monitoring.
Educate residents and their parents or guardians, as appropriate, about medical conditions, medications, and treatment recommendations.
Provide ongoing monitoring and continuity of medical care between physician visits.
Recommend specialty consultations and community resources based on resident needs.
Interdisciplinary Collaboration
Participate in admissions meetings and weekly treatment team meetings.
Collaborate with physicians, psychiatric providers, nursing staff, therapists, and other team members to coordinate care.
Submit individualized medical care plans and goals for periodic physician review and evaluation.
Support discharge planning, including communication of medical follow-up needs to families and receiving providers.
Provide continuing education and serve as a clinical resource to staff.
Safety, Documentation & Clinical Coverage
Respond to medical emergencies and perform emergency interventions within clinical scope and facility protocols.
Notify the supervising or attending physician of unusual findings, unexpected events, or concerns requiring physician consultation.
Document assessments, diagnoses, treatment plans, procedures, and resident progress accurately and promptly.
Follow applicable nursing practice requirements, facility policies, infection prevention practices, and safety standards.
Maintain professional knowledge through continuing education, clinical literature, and facility training.
Perform other duties as assigned.
Qualifications
Education
Master's degree or higher in nursing with completion of a Family Nurse Practitioner program, or a graduate nursing degree with completion of a post-master's FNP certificate program.
Licensure & Certification
Current Texas Registered Nurse license or applicable multistate RN privilege.

Current Basic Life Support (BLS) certification, maintained throughout employment.
