Full Job Description
CHD is seeking a qualified and compassionate Family Nurse Practitioner (FNP) to join our Primary Care team in a full-time, onsite role. The FNP will deliver comprehensive, patient centered primary care services to individuals and families across the lifespan, with a strong focus on preventive care, chronic disease management, and health promotion.

Key Responsibilities
• Provide comprehensive primary care services, including health assessments, diagnosis, treatment, and follow-up care
• Manage acute and chronic conditions in a primary care setting
• Perform physical exams, order and interpret diagnostic tests, and develop evidence-based treatment plans
• Prescribe medications and therapies in compliance with Massachusetts state regulations and clinical guidelines

Schedule & Work Environment
• Full-time, 40 hours per week
• Onsite Primary Care practice
• Monday-Friday schedule with occasional evenings/weekends, and or on call coverage

Compensation & Benefits
  • Competitive salary commensurate with experience $135,000-$165,000 based on experience
